On Your First Visit

 

Upon your arrival to our office for the first time please remember to be at least 15 minutes early so that we have enough time to fill out some paperwork and put together your chart so we can keep you on schedule. 

 

You will need to bring your insurance card, I.D. (if you are over 18), any records from your previous dentist if you have them, and new patient paperwork if you have chosen to fill it out in advance. 

 

If you are currently on medication please remember to bring a complete list of your medications with you. Also to take your medication if your physician has instructed you that you need to be pre-medicated.

 

****If you are more than 15 minutes late for your appointment we will have to reschdule you.  Please call at least 24 hours in advance to cancel your appointment if you are unable to keep your scheduled appointment time. A $50.00 broken appointment fee will be charged if you do not call and do not show for your appointment.****
